A name change, exactly the same as if you were changing your name from Lisa Simpson to Henrietta Willendorf.
You may want to reconsider using a hyphen, though, for otherwise it will cause confusion. Or, at least use 'y' between the surnames, because Joe Michael Smith Jones is always going to look like his surname is Jones and Smith is a given name, while Joe Michael Smith-Jones or Smith y Jones will look more "surnamey".

properly custom is that the little ones take the fathers call properly it is what happened with me as i've got been given my dads hyphenated call. yet some human beings have a tendency to make a mix of the names. i think of we would desire to consistently follow custom and think of of the youngsters as quickly as we initiate making up names. merely take the fathers call!!!!!
