If, during the next 24 hours, the wound becomes swollen, red and more tender to the touch or there is any pus coming out of it, then you need to go see your family doctor and get an antibiotic. Any worsening of the wound in any time frame should take you to your doctor to have it evaluated.

I would get to an emergency room quicker, if not sooner. Rusty nails are extremely dangerous not only for tetanus, but also other bacteria which may be present in the soil which could cause meningitis or the like. Don't mess around !! Get treated immediately. One never knows if the shot you took is still effective.

If the nail does not require removal, simply clean the wound very well, use an antibiotic ointment on it, and put a bandage on it for now. Clean and apply ointment as per manufact.. instructions. Keep an eye on it and watch for infection. If it becomes red and hot, go to your GP.
You will not contract tetanus, the vaccine is now recommended once ever 10 years. Just keep it clean, apply something like triple antibiotic ointment, and watch for infection

You could get an infection that can spread into your blood. That nail could have other nasty things on it.

Yes ...you need antibiotics..there is a nasty bug(pseudomonas) that lives in shoes,feet etc..that you have now injected into your skin and heel tissue.
