It's called an Advanced Medical Directive. It gives your doctor treatment options if something were to happen.
A Living Will is a document that gives a person, who you designate, the authority to make medical decisions for you in the event that you are incapacitated.

This is a living will. Also, if you don't have one, you're family (if they know your wishes in advance) and request a "no Code" on you. This means the doctors won't call a code blue and try to bring you back.
In fact, once your comatose, your family has a lot of authority. So, until your living will is done, make sure your family knows what you want.
