I know its $45 but does that include tip money and tolls?

All wrong guys .There is no flat rate TO JFK only FROM JFK and it includes neither tips nor tolls [if applicable]

No. Typically a ride from Manhattan to JFK will cost $55-60 after paying tolls and tipping the driver

It includes the tolls, but not the tip. Tips should be anywhere from $1 to $5.

Flat rates never include the tip. The tip should include $1 per piece of luggage or if you have no luggage it should be 10%. so you would give the cabbie $50 including tip. Keep your receipts as they are tax deductible if it's a business trip.

Tips are never included in all inclusive deals, the only time a tip is included is when you have more than 10 people at the table in a restaurant/bar.

The best and cheapest way to get from JFK to 622 Broadway, which is between Bleeker and West Houston Streets by public transit. Step 1: take the Airtrain from the terminal to Jamaica Station. Please listen for the announcement "Station D: Jamaica Station." Step 2: at Jamaica Station, purchase a $7.25 MetroCard from the MetroCard Vending Machine, and exit the Airtrain system into Jamaica Station. A $5 fare will be deducted from your MetroCard, when exiting the Airtrain system at Jamaica Station. The remaining $2.25 will use for the subway. Step 3: Once thought turnstile (faregate), follow the directional signs to the New York City subway. Upon entering the subway, take the Manhattan-bound (westbound) "E" express train (Blue Line on the subway map) from Sutphin Blvd-JFK/Jamaica Station to West 4th Street-Washington Square. Step 4: at West 4th Street-Washington Square, transfer downstairs to the "B", "D", "F" or "V" trains (Orange Line). Take either Brooklyn-bound (southbound) train from West 4th Street to Broadway-Lafayette Street. 622 Broadway is a block north of the Broadway-Lafayette Street subway station.
