Have you had him tested for mange mites? Oh... sorry, I read staph infection. How long has it been? Know what, I'd still take him back for a mange test, which is really a series of shots one week apart (2 on average 3 at the most) and here is what happens, it it gets better then it was mange... if not, then it wasn't.
Do not be afraid to go back to the vet, because they could have been correct in part.
So.. give that a try, it could be that your dog has developed an allergy of some sort and needs a little time to recover from it.

Try a different vet, perhaps one that practices integrative medicine (holistic and conventional). Sounds like what your vet did is treat the symptoms, if you haven't done anything differently, he's still subject to the same problem. You need to figure out what caused the problem in the first place. It may be as simple as changing his food to a higher quality diet. Especially if your feeding something like Pedigree or Dog Chow that has a lot of fillers and artificial colors and preservatives.
Changing the food would be the first thing I'd try. Nutro or Blue Buffalo is available at most PetsMart. At Petco, you can get Nutro, Solid Gold, or Natural Balance (I'd go with Natural Balance they have several allergy formulas that are good). It's not uncommon for dogs to do well on a food for years, and then become intolerant of it for whatever reason.

Sounds like you do need a second opinion. And it does seem like he could have mange, they do a skin scraping for that and sometimes they dont show up even though the dog has it. Been there! As for shampooing.... it can make itching worse so be careful in your choice. Here is link to my fav home remedy site and I say from experience that Ted's home remedy for mange helped my dogs skin almost immediately and the itching was almost gone by the 3rd treatment. It didnt cure her mange but it made it more managable until I could get to the vet, her hair even started growing back. I bought all the ingredients at Walmart for about $5 and the 20 mule team borax is in the powered laundry detergent isle.
When I got to the vet she had mitaban dips and antibiotics costed me nearly $500 for mange meds and medicated baths and skin scrapings...But knock on wood she has been mange free for almost a year. good luck!

Why are you afraid to go back to the vet?! They need to know if you still have a problem, otherwise how can they help??! If the vet in question is so terrifying, find another that you feel more comfortable with - but don't expect to find answers on here. We're not vets, and no one can diagnose over the internet.
If the vets don't hear from you, of course they'll assume the problem is solved. It's not a crime for the initial medication not to have worked!!

A staph infection (bacterial) is usually a secondary infection. Don't be afraid to go back to the vet, that's the only way you'll get to the bottom of this. One thing I would try is a grain free lamb diet, your dog maybe allergic to something you're feeding him. Good luck.

I would get a 2nd opinion on this one. To me it sounds like Flea bite dermatitius but you said he has always been on front line. It could be a reaction to the front line. Stop the front line put him on revolution. If he is on Heart guard then he can stop that too on the revolution. It might be a skin infection that is just being stuborn to get rid of. I would seek a different vet on this one. get a copy of his records and take with you.

My canines gets ill (throws up) between Aug - december. we predict of it rather is an hypersensitivity as we've had tonnes of assessments executed and each little thing has come decrease back widespread. It does sound like it would desire to be an hypersensitivity. would desire to that is a warmth rash? reliable success, let us know what it seems to be!
