I'm so sorry you're feeling lost and alone. I, too, have been there.
You need to focus on one day at a time. Get through each as it comes, while on the other hand expect that someday things will get better. If you're feeling at the bottom, there's only one way left ... up ... so what do you have to lose?
I managed it, and am glad. I am now the mother of three grown children, and the grandmother of seven. Had I not plodded through the hard times, I would've missed so many good ones, and perhaps wouldn't have appreciated them as much either.
The bad times are there to strengthen us ... to help us become more resiliant. My daughter was having a bad time in school, being harassed by other students. I told her that as long as she was being a good person, doing what was right and good, and working as best as she could at whatever was put before her, that it was the other people with the problem ... that she should hold her head up and keep on keeping on. Her sinking grades changed back into high ones, and she went on to achieve great things in speech, theater, ect.. She has since graduated college and has become an educator. Her early lesson in life paid off ... not to give up.
Have the courage to forge on! Even if there are failures, a person can learn from them ... perhaps moreso than they can from their successes.
I hope there is something here that will help you!
Best Wishes!

Maybe take a day off. (we used to call them "mental days".) Sometime's it'll get you over the hump. Try to exercise. It'll get your blood flowing into your brain and maybe take away some of the stress and depression. I'm sure that not EVERYTHING you do turns out bad. It's just that you focus on those that DO turn out bad. Do you have a friend that can help you "carry the load". ? Maybe you could go to the school counselor. Sounds like you're really depressed. Are you eating right? Get on some vitamins...they help with stress..esp. certain ones (I can't remember...I think the B vitamins) You need to "build yourself up" so you CAN handle the things in your life. Wish I knew more to tell you . Just know that A LOT of people go through this and come out of it. Good Luck.
EDIT..Also get some Omega 3 pills (google omega3 and look at all the things it's good for.) It doesn't cost much..you can get it at WM. or any drug store.

"Everything"..."always".... those are absolutes and I find it hard to believe that you are that much of a loser. Maybe a few things go awry, but not EVERYTHING ALWAYS.
We all make mistakes and we hopefully learn from them. We need to make wise choices, consider how things will affect us and others in the future.
You are having a great pity party. Stop it! Begin right now to change things, you can do that. Every day brings new opportunities to change, do it now.

Seek help. If you're in school, there should be a guidance counselor or some adult you can trust. It sounds like you're feeling overwhelmed, which happens to many of us. There's no reason you need to carry the burden alone---And schools provide people who are there to help students who feel this way. Take them up on it; ask for help.
