It absolutely doesn't mean that - what's the context you've come across this in? If it's in relation to personality disorder then it refers to having very easily changeable emotions and having some difficulty controlling this - which can be very distressing and exhausting. It definitely doesn't mean someone is a 'psycho'.

People can be emotionally labile and show emotional dysregulation and not be psychotic. Not being able to manage your emotions can mean that your emotions fluctuate wildly in response to environmental stimuli or that you react too strongly to environmental stimuli. As people mature, one sign of maturation is the ability to manage emotional reactions and to respond more rationally and with reason. There are few situations in life that require strong and immediate and strong emotional reactions, like a killer instinct or a strong fear and flight response.
People who cannot manage their emotions may be drama queens, but not all of them are dangerous or psychos, just unpleasant people to be around and people you may find difficult to trust because you don't know how they will react. Of course it depends on if it is a temporary condition or a permanent long-standing one.

It simply means you are unable to control your emotions either temporally ( like from hysteria) to permanent (like what you see with people mentally ill)
For example if your mom yells at you and you lash out at her and start crying and yelling that is unstable emotionally
