There is no getting out of it,, I paid SS taxes for 30 + years and I will only be able to collect on them at age 67,, May as well get used to it my friend,, The government is also good at borrowing the money from that fund to fund other things and they never pay it back,, It is called legal theft in my opinion,,

I not think there an actual form. It probably required people working pay.
Now, if your Self-Employed you can not pay depending on which line you place your income. But, you not pay you not receive if/when you retire.
I can not promise we have Social Security in 20 more years. How I see it is for the ones who already drawing, our Elderly, who have contributed already. I hope there may be something for a person, so, I hope I get a return for my money. I not going to expect it and will try to save by other means (Which Social Security has stated for decades). Maybe by that time they may not be able to give a whole lot of money, but, maybe they could at least give the retired Tax exemptions like Sales Tax and Property Tax etc. That alone can add up to a Substantial return, if a person own a home, car, etc. and buys food and clothing.

Become a teacher in the public schools. They pay Medicare but not social security. You can also become a minister or join the Amish.and opt out there. Other than that, your options are few.

Of course there is no such form, nor is there any way currently to avoid paying into the SS system, unless you have one of those rare jobs where the employer has opted out of the SS system legally.

Paying social security taxes on earned income is not voluntary. Your employer is required to withhold them from your salary and pay them to Uncle Sam.

There is one but it is only for members/ministers for organized religions and only applies to their wages from the church.
There isn't one for the rest of us.
