He could have Juvenile Periodontal Disease. It is the same gum problem that older people get but it turns up abound 12 years old and attacks many teeth at the same time.

He should have his dentist refer him to a good periodontist. If the dentist offers to pull all those teeth he should get out of there ASAP and find a better dentist. That guy wants to sell him implants at $3200 each and is probably not even qualified to do implants.

A good periodontist would scrape off the decay, give him some initial antibiotics, and then give him a long term prescription for 20mg Doxycycline which will encourage bone growth to anchor his teeth tighter. There are some other treatment details that a perio guy could explain. His problem is treatable and does not have to cause loss of his teeth.

Please give more information. How old is your brother? How many teeth are loose, and how long have they been that way? Have any actually fallen out ( other than "baby teeth")?
Could be poor oral hygiene, gum disease, or rarely some systemic disease. Perhaps Langerhans Cell Histiocytosis ( i just had to throw that one out there- it's pretty rare).
His dentist should be able to answer this question .

I can tell you the dentist will not judge him, or if so he needs to find a different one. Could be lack of calcium in his diet, I hope he's not using drugs, but some street drugs interfere with nutrition, could be neglect of his teeth and gums. I hope a dentist or someone with a similar experience comes along and gives you a better answer.

Never is loose teeth good. And when your brother saw the Dentist 2 years ago didn't he tell him then what the problem was because this kind of problem doesn't happen over night.
