depends on the brand. But normally the thick white one is called coconut cream, and the thin one is coconut milk. If you're using an Asian (Thai, Indian, Filipino, etc) recipe, go with the thick white creamy type

Coconut milk is white and fluid, a lot like cow's milk. Usually in a can of coconut milk there is the coconut milk with a layer of thick coconut cream floating on top. The better the brand the more coconut cream there is. The best brand I have found is Chaokoh but if that's not available look for a higher fat content in the nutrition data. It's not worth it to buy lower fat coconut milk because that's just regular coconut milk that is watered down.

As everybody says, it will probably be seperated if you don't shake it before opening. The fat does not however settle at the bottom, it floats to the top just like cream in fresh milk.
And coconut milk is not the liquid found inside a coconut. It is made by grinding and cooking the meat from the coconut then straining the liquid.

If you don't shake it before opening the can, it will be separated into two layers. The first will be clear and oily while the second will resemblethick yogurt. If you shake it before you open it, it will look like thicker evaporated milk with a very slight coconut smell.

It's not all smooth an homogenized the way cow's milk is. It's normal for some liquid to seperate and rise to the top, while the fattier part settles to the bottom (you have to scoop it out). As soon as you heat it it will all melt together.
