Use head lice shampoo, thats what I use when I wash rescued puppies instead of flea shampoo. It works much better.
Head lice shampoo actually has the same exact stuff in it, except head lice shampoo has more, and flea shampoo has more water. When I used to use flea shampoo, I usually had to use it more than once on the same dog/puppy. When I use head lice shampoo, I only need to use it once and the fleas are gone.
The worms are called roundworms heres who to treat them at home.
1. Pick a large handful (or more) of grass that is more than about 8 inches long. Mix it with a can of dog food or raw hamburger. Make sure that the grass is coated in the food.
Feed it to your dog like that.
How it works: The hungry worms go straight for the food and get tangled/caught in the grass. The more the more they try to escape, the more tangled they get. The come out with the grass when the dog poops it out.
2. Feed the dog grease (from cooked bacon, duck, goose, etc), and/or lard.
How it works: The grease makes the dog's stomach and intestines greasy, which causes it to be very slippery for the worms. The worms have nothing to hold on to and come out in the poop. The dog may have greasy-watery poop, but the worms will be gone.
The vet has pills for roundworms too.

NO more Hartz please! It sounds like you need to look beyond just getting rid of the fleas and find out why your dog is so attractive to the fleas in the first place. Fleas are scavengers of the immune system and animals that are ill, malnourished, or unhealthy have more fleas. Please look into what you're feeding and get a better food if that is an issue. Dogs are carnivores and the first ingredient in the kibble should be some type of meat as opposed to a grain or a meat byproduct. Also, overvaccinating may have caused him some problems as well.
Here is a good way to rate your dog's kibble:
How to grade your dog's or cat’s food: (Some brands are done at the very
bottom)
Start with a grade of 100:
1) For every listing of "by-product", subtract 10 points
2) For every non-specific animal source ("meat" or "poultry", meat,
meal or fat) reference, subtract 10 points
3) If the food contains BHA, BHT, or ethoxyquin, subtract 10 points
4) For every grain "mill run" or non-specific grain source, subtract
5 points
5) If the same grain ingredient is used 2 or more times in the first
five ingredients (i.e. "ground brown rice", "brewers rice", "rice
flour" are all the same grain), subtract 5 points
6) If the protein sources are not meat meal and there are less than
2 meats in the top 3 ingredients, subtract 3 points
7) If it contains any artificial colorants, subtract 3 points
8) If it contains ground corn or whole grain corn, subtract 3points
9) If corn is listed in the top 5 ingredients, subtract 2 more points
10) If the food contains any animal fat other than fish oil, subtract
2 points
11) If lamb is the only animal protein source (unless your dog is
allergic to other protein sources), subtract 2 points
12) If it contains soy or soybeans, subtract 2 points
13) If it contains wheat (unless you know that your dog/cat isn’t
allergic to wheat), subtract 2 points
14) If it contains beef (unless you know that your dog/cat isn’t
allergic to beef), subtract 1 point
15) If it contains salt, subtract 1 point
Extra Credit:
1) If any of the meat sources are organic, add 5 points
2) If the food is endorsed by any major breed group or
nutritionist, add 5 points
3) If the food is baked not extruded, add 5 points
4) If the food contains probiotics, add 3 points
5) If the food contains fruit, add 3 points
6) If the food contains vegetables (NOT corn or other grains), add 3
points
7) If the animal sources are hormone-free and antibiotic-free, add 2
points
8) If the food contains barley, add 2 points
9) If the food contains flax seed oil (not just the seeds), add 2
points
10) If the food contains oats or oatmeal, add 1 point
11) If the food contains sunflower oil, add 1 point
12) For every different specific animal protein source (other than
the first one; count "chicken" and "chicken meal" as only one
protein source, but "chicken" and "" as 2 different sources), add 1
point
13) If it contains glucosamine and chondroitin, add 1 point
14) If the vegetables have been tested for pesticides and are
pesticide-free, add 1 point

OOOH umm worms in urine is NOT normal, I've never seen that and I've seen some pretty bad cases of worms, THAT means vet time. Plus Hartz sucks. Your going to have to buy frontline or advantix, those are the only two that work, and in some areas one will work and the other wont depending on what fleas have built and immunity to. You also need to have your dog checked for worms and wormed, fleas carry tapeworm eggs and they steal nutrients from dogs, your dog can literally starve to death if you dont get him wormed.

First off are you crazy? No wonder he has fleas! This is the stuff you got from vets years ago! Hello! The fleas are now immuned to this stuff, and also I despise hertz and sargents products they not only cus me to have a serious allergy attack but the animals you use this stuff on it can KILL them!
And are you sure those are worms?
You are more than likely seeing crystals in his urine which is a clear sign of infection, these are the only thing that come close so my advise is to take the dog back to the bathroom, using Dawn dish liquid.
This will get off oil animals that have been in major oil spills, as well as getting the chemmicals off your dog. The only othe dish soap is Joy mild, do not use human products on them as this is acting like a Lab Tech to your dog, you wouldn't want these labs testing on him would you?
I would get into the Vets like possibly today or take him to your regular vet in the morning or even another vet, OR your local Emergency VET center to get this dog checked, because this is serious with the Urine.
Also talk to your vet about which flea rid method is right for him, The vet will be the one to decide.

Don't use flea shampoos, they don't work. Nothing from a pet store does - stores aren't licensed to sell the appropriate chemicals for flea control. DEFINITELY do not use Hartz products!!
Get some proper treatment from a vet, like Frontline, Revolution, Advantage or Advocate. Sounds like you also need to get him a wormer. You don't get worms in urine, what do they look like exactly? If your dog hasn't been to the vets in the last year, take him for a check up. I don't know what these worms could be you're describing, unless they're around his anus - if they're actually in the urine, they're not worms.
