I understend QRS , but what specificly height of QRS means? At rest it is shorter than after exercise , whay?

To start with.....The 'QRS' complex of the ECG represents ventricular depolarization, causing the ventricles to contract. The 'QRS' complex can be used to determine a patient's cardiac rate. The QRS complex tends to look "spiked" rather than rounded due to the increase in "conduction velocity".

The height of the QRS referred to as the AMPLITUDE corresponds with the "VOLTAGE" conducted with every contraction of the ventricles. As why it is shorter at rest than after exercise, obviously because during exercise the heart function is increased which means there is more VOLTAGE upon ventricular contraction.

The height or amplitude of QRS has wide normal limits. An amplitude of 5 mm or less in all three standard leads is considered to be unhealthy often seen in coronary disease, cardiac failure, emphysema, and obesity.
The upper limit is variable from lead to lead, but generally is between 20 and 30 mm. Amplitude are affected by many factors besides the health of the heart. Some of these factors include chest size, chest wall thickness,and emphysema -these factors should always be given consideration before judging the voltage of any complex to be too high or too low.

A widened QRS would, indeed, be consistent with V-tachy. Neither a-fib nor a block would lead to a wide complex or prolonged QRS complex. Of the three choices you mentioned, I'd have put wide complex, ventricular tachycardia as the focus of the pacemaker in the ventricle instead of the normal "electrical highway" hence the relatively slow, widened QRS complex. In advance cardiac life support it's critical to identify whether a tachycardia is narrow or wide complex. A widened QRS is most commonly the result of a drug and doesn't have to be in the setting of something horrific, but it's when the focus of the pacing is below the AV node.

To oversimplify the previous answer- the height of the QRS is indicative of the strength of the electrical impulse. If there is something "solid" between the heart and the sensor, such as fluid around the heart, the amplitude will be low. The answer about activity is right on.

Also, by looking at the patterns of the amplitudes of the QRS from different leads( Which look at the electrical depolarizations from different angles) , an experienced interpreter can "see" the way the heart is lying,a nd the way that electrical impulses are traveling, and make certain diagnoses

Prolonged QRS represents slower than normal depolarisation of the ventricles. This most commonly happens due to bundle branch block, but can also happen in hyperkalaemia and tricyclic antidepressant overdose. Ventricular tachycardia is another cause of broad QRS complexes, but these complexes typically have a very different morphology.
