The Mother of God and of all Creation cannot die, She is the Great Mystery and manifests as all forms. In Her specific form as the Mother of Jesus I believe she left that incarnation in the same way many great Tibetan saints and yogis have, by assuming the rainbow body i.e. transforming the physical body into light. No, Jesus tells the Apostle John to take her into his home, we can only assume she stays there until her death. Her family would have been under tremendous pressure from the pharisees and the sanhedrin court. The new testament and the new covenant is what Jesus was all about anyway, Mary was human just like us and a sinner just like us, by the way she was not a virgin after giving birth.

No we have no further accounts after the crucifixion. However, traditionally her tomb is in Ephesus where she reportedly lived with John the Apostle.
There was no assumption of the perpetual virgin if that is what you are driving at. She had several other kids which is attested to in scripture (James and Jude were two of her kids).
edit-- ah yes I stand corrected Acts 1:14 as well (by the way this mentions Jesus' brothers as well in case the perpetual virgin thing is still being considered)

Not Biblical. There is a writing called the Assumption of Mary which is not believed to be inspired, not factual.
I cannot recall all details. I do think the couple of best answer people here are Catholic, and I'll bet they have information regarding this.
