Start with three coins: a penny, a nickel and a quarter. Say, "Tommy's mother has three children. The first child's name is Penny." (Hold up the penny while saying this.) "The second child's name is Nick." (Hold up the nickel.) What is the third child's name? (Hold up the quarter.)

Insufficient data.

The third child could be named anything.
It is possible that it is "Tommy" ... but then it is also possible, given the conditions of the question, that "Tommy" is an adult, thus not counted amongst his mother's "children" due to being over the age of 18.... which leaves the position of the third child open.

It all depends on how old Tommy is, but since we don't know that then the question cannot be absolutely answered.
