It's because of a bacteria called (beta)-galactosidase. It inevitably builds up in milk over time. I would say if milk is left out in room temperature for even a day, do not drink it. Boiling wont make it any better to drink after that, it sounds like a nasty idea. The main point is to just keep your milk in the fridge when you're not using it.

I go by smell. If the milk smells okay then put it in the frigde and have a good day. If it smells a little icky, pour it out.

NO! milk should be out of the fridge for 2 hours max!
7 hours?! thats like going to work and coming back and the milk is still marinating on the counter... gross! toss it out!

depends what kind of milk it is, check the carton, because there is a brand out there (boxed milk) that does not require refrigeration until you use it; but if it is your standard 'plastic jug' milk, then yeah it is bad

If it doesn't smell then take a sip. You should be able to smell it or see chunks floating if it is bad.
