Depends where you rent, and what kind of bike.
At the bike parking of the railway stations you can rent a basic, (fixed speed,) bike for € 6.50 ($9.50) a day, €32.50 ($47.5) a week.
For a bike with gears, and brakes on the handle bar, you pay €8.40 ($12.25) a day, a weekly rate is not mentioned on the site but might exist.
On top of that you need to 'pay' a guarantee, between €50 ($73) and €150 ($219), but you will get that money back when you return the bike.
Locks and insurance are included in the price.

Amsterdam Centraal Station, bike parking, does have bikes for rent.
