There can be many causes.
1. Certain medications
2. Drug abuse
3. Genetic abonormality (inherited or random)
4. Early malformations
The first two situations, of course, are within our control, but genetic mutations happen at conception, and early malformations in the heart occur at about 30 days past conception.
Our third baby was born with severe heart defects. After testing we found out she had a genetic disorder called Charge Syndrome, which means her heart problem began at conception, at the chromosomal level.
It was totally random and does not run in our family, so we do not think it would happen again.

Usually some kind of birth defect or genetic disorder. If this question is about your child that you are currently carrying then you should ask your doctor for more specific details regarding your situation. I dont think anyone here would or should attempt to tell you what caused a heart problem without all the information.

Hi there. I specialise in cardiac ultrasound, and many cardiac abnormalities are simply abnormalities of development. There is often not a 'cause' or 'reason' that the heart did not form properly. Examples of this are when the heart chambers rotate the wrong way, or when tissue is reabsorbed leaving little holes between chambers. The amazing this is that despite an abnormality most of the children cope really well when they are managed by a cardiologist once born. Hope I have helped you.
