Urine is actually rather clean, unless there is a bladder or urinary track infection. As long as the urine is not infected, there is no danger to the woman. In survival situations, urine is safe enough to drink for hydration purposes.

Urine can be very toxic, but it is also a very good cleanser, though I wouldn't recommend ever urinating in a womans vagina. The woman would eventually evacuate it.
