I've had the same question for a while.
No it cannot mean a dick becuase i have seen several teachers use it, so it can't be rude or desperately degrading.
As far as i can tell it means that someone is always willing to help and never lets up, they always follow the rules no matter the incident. I'm probably wrong, but thats what i've picked up.

When I was going to college, a tool was a guy who studied to the exclusion of other social or athletic activities with assumption that he wanted to do better than everyone else. I never heard it defined but the context was like using the word "fool" but since the person was sort of being contructive, he was like a tool - good for only one use.

I think it's a metaphor for the male genitalia. Calling someone a tool is the same as calling someone a di*k.

It is a put down. Like most put downs it get used in a variety of ways, It can imply that the person is stupid, thick as a brick or the person allows himself to be used. While it also refers to male genitalia, I think that is a separate unrelated usage.
