The LC spelling of the name of the composer of trombone studies is
Vladislav Mikhaylovich Blazhevich; (b. Aug. 3, 1881, Tregubovko; d. Apr. 10, 1942)

Per LC: Blazewitsch, V., and Blazhevich, V. SEE Blazhevich, V. (Vladislav),

The Warner Bro./Alfred publication "Classic Festival Solos (Trombone), Volume II Solo Book" has the spelling as
Blazewitch, B.[sic V.] M.

Publisher: Warner Brothers Publications, Inc.

Some music publications use the spelling Blazevich which is no longer accepted by the Library of Congress.

I have Googled this quite extensively and can find no records of his birth/death dates. I can't even find what BM stands for. Sorry the mystery continues!
