Yep. One of the first games I remember was the Detroit Lions in 1970. A receiver i think named Chuck Hughes collapsed and died during the game.

Yes. Players have collapsed and died in extreme heat...Kory Stringer of the Vikings died during practice back in 2001...

Not sure I understand the question. But there have been active players killed in car crashes, shootings, other accidents,drug overdoses, players have had diseases they died from... if that's what you are asking yes.

yes i no there have been people not sure who i think that dick butkis (worst name ever!!!) put someone in the hospital who died shortly after
