Well, if you think of one man who headed up a "family" made up of those from his hometown, you have to consider the late Saddam Hussein. He was the "thug from Tikrit" who hijacked the Ba'athist political movement, filled the government ranks with others from Tikrit and plundered the national treasury in the process. He also may be the only gangster who carried out a massacre (the gassing of the Kurds) which makes Capone's Valentine Day's Massacre look like chump change.

I would vote for Pablo Escobar. He even bombed an airliner full of passengers, women and children included to kill one man he did not like. Everyone on board was killed. He lived by the gun and died by the gun and good riddance.
