Either one. In Pakistan's case, they probably manipulate the elections. Because after all, Pakistan is an American ally. It looks better for the U.S. if it looks like an actual election's results are respected.

free elections make the people believe they have some sort of control.
By allowing them to choose between two options it makes them feel like they aren't being controlled.
