If the layout is just one color, the screen should cost anywhere from $50 to $100. (shop around for the best price. This job is so simple, it would be hard to mess up)

After that, the cost depends on how many shirts you get printed.

The first shirt, in other words can cost you $100, but each shirt, after that, may only be five or ten bucks each.

Check your local listings for screen printers and ask for prices. The prices usually go down the more you order, and there is a one time set-up fee and price is also effected by how many colors you want, but if you look around you should find a good price.
