This is really difficult. What is the membership form for? host and guest sound a bit like a party but if it is for a gym or something like that it obviously needs to be a bit more formal.
My mind has drawn a blank on words but how about welcomer or something like that.

By joint membership I assume something like a man and wife or two good friends.
Does it really matter?!
I imagine you are doing this because either
1- You have had a complaint from a 'guest' claiming they don't feel equal to their friend/wife/husband/other or
2- Someone has nothing better to think about.
Either way it's barmy!
However, maybe client, patron, member or nothing at all, just their names?

Client 1 and 2. Sounds a bit more upmarket than friend 1 and 2. It is difficult to use just one word to differentiate between 2 people.
