very old question, but maybe the reason is even with the needle in the arm, there is a possibility for a stay of execution. If the alcohol wasn't applied in this situation, the arm could get infected, and lead to health problems for which the state would be responsible.

Because the one who is administering the injection is a qualified "doctor" (of death, lol), it would be considered malpractice and un-hygenic of the doctor, which is illegal and would be means to have his license(s) revoked.
Remember the one that was injected but would not die? Well, if he (or anyone) was granted an extra day of life and die of an infection, well, you know the rest. There'd be a huge uproar, blah blah blah. You know damn well the lawyers would be after them!

I've just excepted that as a question I will never know a true answer to. My ex boyfriend was in prison and one time he asked a gaurd and the gaurd just ignored him and this was a gaurd that normally would talk for hours with the inmates he liked.
