Italy has La Cosa Nostra which is still feared all over the world.
The Russian's are different.They are feared because you can't take them down.they have many powerful groups but not one main family.Even if you take out one big group of the Russians they are many and will replace what goes missing straight away.
La Cosa Nostra has to be the most powerful though.Them and the Russians both use unorthodox methods and are very un-humane.

I wanted to say Italy but truthfully it has got to be Russia. The Russian mafia has reaches into their military and other parts of government.

Russia, because of the black market after the fall of communism. And the peoples ability to view these practices as acceptable behavior in their society.

Russia. I think this way because the Russian government seems to be powerless to do anything about them.
