It is just immaturity. The narcissist is still a baby. Babies are fully centred on themselves and their own needs without any thought about others. As they grow up, they are supposed to learn about considering others as well. Narcissists don't ever learn to get their eyes on anyone else other than themselves.

My son (now 50) was taken away at age three by his alcoholic father and hidden from the family until he was 30. No one knew where they were and he was never told he had a mother. He suffered much abuse--physical and emotional, as well as neglect. They were always one step ahead of child protective services. He often had to beg strangers for food. He was constantly told he was worthless and wouldn't ever amount to anything, but he has risen somewhat from this indoctrination, but it's resulted in his having low self esteem and a weak ego. He is a textbook example of Right Man Syndrome with some narcissistic qualities. He has a 13-year-old daughter who he worships, and for whom he greatly overcompensates for his miserable childhood. In other words, she is utterly spoiled and definitely narcissistic in her behavior as described above. He will not allow anyone, even her mother, to have much authority over her rearing. It would be no surprise if she turns out to have Narcissistic Personality Disorder.

In some cases, it might have to do with parents who think too much of their children without giving them reality checks. (Especially if it's an only child). Beyond that, I don't think a lot is really known about the causes.

there may not be a cause, may be it is just how that person is, it being a disorder because it is not a desirable way to be
