make or break up?

i'm diane,17,in a relationship.. we've been dating for 4months now,at first i was really in love with him.even up to now,but the difference is we're both no longer that active in our relationship..we dont see each other from time to time now,unlike before..i am always the one waiting and spending lots of time contacting him..i think im already falling out of love because of these issues.. what should i do? make or break up?
