I think it's a good question, actually. I kind of wondered how other people drank too.
When I drink it's almost always inside, especially if I'm around members of the opposite sex, because I noticed back in high school that some guys actually look at that (both lips on outside) & think it's a sign that you go down...silly I know, but it makes me self-conscious now. I guess there's some things that you just carry with you, no matter how silly. =]

Outside, but sometimes I tuck my upper lip inside because the bottle makes this weird "sucking" noise when I do it the other way.

I drink on the inside because it feels uncomfortable on the outside and I tend to end up choking on the water by doing so.
