Now this is a tough one 'cos the noisiest and 'most guarrenteed to annoy the hell out of the parents' toys are usually battery operated these days and can be easily silenced by removing the battery and I know a few parents who 'forget' to replace dead batteries......
So I'd suggest looking for a traditional toy, when my little sister was about 1yr our grandparents bought her zylophone that used to drive our mum nuts

Walgreens has a squealing Christmas Suited Piglet that is very rude and the young one's love it . Fuzzy enough for a 1 year old .

One of those battery operated pianos at the Dollar General for $10. If, you get lucky you may find one with a volume control that broke on high and get it for half price.
