Is it true that she's a spider mutated to a superhuman?

No. But it was the first idea of the writers. Though her mind was manipulated and she is convinced that she's an evolved spider.

Yes and no. Spider-Woman's first appearance was in Marvel Spotlight #32. In that story, she was evolved from a spider by the High Evolutionary just like the rest of his Ani-Men. In Spider-Woman #1, her origin was changed since the writer (Marv Wolfman) thought that the original origin was too creepy and needed to be changed if she was going to remain a popular character.

This is how the new origin story from Spider-Woman #1 went. Jessica was actually injected with an untested serum created from the blood of various spiders by her father because she had gotten very ill due to the uranium around their home/laboratory near Wundagore. The four year old Jessica was placed inside a genetic accelerator to help her heal faster, but the process would only work at a slow rate. She remained in stasis for decades and finally emerged from the genetic accelerator as a 17 year old with no memory of her past. An agent of HYDRA known as Vermis took in Jessica and trained her to be HYDRA's deadliest assassin. She ended up rebelling against them and eventually killed Vermis who tricked/brainwashed her into thinking that she was a mutated spider just before he died.

This may not be the actual reason for it, but I think it basically means that Marvel Spotlight #32's origin was actually a flashback of what Jessica had thought was her origin was and she remembered the truth in Spider-Woman #1. I don't recall that ever being stated in any issue or any interviews though although I think it is a pretty good theory to tie the two together.

The miniseries "Spider-Woman: Origin" changed her origin around once again. Instead, Jessica had been exposed to a laser beam containing the DNA of several spiders while she was still inside her mother's womb. Several years later Jessica woke up from a coma at the age of 17, and she was told that her parents had disappeared/died when she was found. She was trained to become an assassin by Taskmaster after being recruited by HYDRA. Vermis was retconned into being an old retired HYDRA agent that Jessica seduced in order to lead her to her mother.

The other three Spider-Women were all normal women that were genetically altered using spider DNA by various people.

Jessica Drew the first Spider-Woman is the only one who's origin I know. As a Child she got really sick and her father a scientist used and experimental cure he had developed to cure her. It worked but only after putting her in a state of suspended animation and she had her powers when she awoke. Her father was working with a villian known as the High-Evoutionary at the time.

At one point yes, that was true. In her first story they revealed that was her origin and it grossed her out. Fans and creators were apparently uncomfortable with it as well. So it was retconned (meaning they changed it) and it turned out it wasn't true.
