If you tell your staff to follow such a standard, aren't you also saying "perception is everything". So the "office psychopath"-in-training will simply give the perception that everything works. Too bad if your organisation is trusted to produce safety-critical systems.

Absolutely. One prime example is in the food industry. Food that is served must have "presentation, presentation, presentation."

Yes, they do. Having said that, I sincerely hope that organizations trusted to produce safety critical systems are doing far more than creating a perception of "everything works"... dangerous indeed !
