It depends on the state laws.
In my state, I am required to break confidentiality if someone says they have an imminent plan to hurt themselves or someone else. I am also required to report child and elder abuse.
In another state where I practiced, I was also required to report childhood sex abuse, even if the patient is an adult now.
Of course, I tell everyone this upfront before I even begin the session.

A psychologist is required by law to report if you are a danger to yourself or others, or even if you are talking about being a danger to yourself or others. They tell you this right at the beginning of your sessions with them, so it doesn't actually fall under the umbrella of confidentiality.
