The Bible as we know it today was put together in or about 1609 by the monks of King James of England. There were earlier versions that were compiled during the early part of the rennaissance. Prior to that, the various books of the Bible were read separately by the priests and monks of the Catholic Church. The word "bible" comes from Latin meaning library. Thus, the Bible was a library or compilation of many books of sacred writ that started out separate and became one volumn over time.

Council of Nicea canonized the 66 (62, actually) books that we call the Bible in the year AD325.

They actually canonized 7 additional books and portions of 4 other books, but those were removed by the Protestant Reformation (still included in the Catholic Bible) in the 1500's.

And I say 62 instead of 66 because until that point, the books of Samuel, Kings, Chronicles, and Ezra were single books (Ezra is now the books of Ezra and Nehemiah).
