The 777-200 ER carries 45,220 gal. of fuel, can can fly 7,700 nm... that is .170 miles per gallon... not a very large increase... therefore, it is mostly the added capacity, but economy helps as well...

Lets take the 777-200ER

Using these figures with 45220 gallons of fuel the 's range is 7700 nm or 8,855 standard miles. this translates to 5.1 miles per gallon. however airlines look at passenger miles so with 200 passengers the fuel usage drops to 0.0255 gallons per passenger mile

The additional 14,220 gallons on the 777-200ER gives it the extended range. And since the fuel is stored in the wings there has to be additional structural strengthening of the wing and landing gear in order to support the additional 92,300 Lbs of fuel

It depends. Extended range jets carry more fuel, and burn more fuel to take off and cruise the first part of the trip because they have to haul that extra fuel for the extended part. Then again, if they can make the trip in one hop instead of two because of the extra range, they save all the extra fuel it would have taken to land and take off again in the middle of the flight to refuel.
