There is a company called "Nielson" that signs up families for 3 month stints. They either ask the family members to fill out 'diaries', and they also have a device that includes wrist-bands for the family members to wear. The device notes who in the family is watching what show.

While there are other ways of doing this, Nielson is the industry standard and Millions (perhaps Billions) of dollars rest on these numbers so they are watched very closely and kept very honest.

The simple report of ratings come out Wed morning and newspapers often carry a 'Ratings' number for shows in the Wed edition.
