A lot of the detail tells me this is a doctored photo, but, I think it MAY be anatomically possible, since a tear duct empties out into a sinus cavity If a hole in an earlobe be enlarged, by stretching, it's conceiveable that the same can be done with a tear duct.

It can be done in real life OR on a computer. It's just like when someone sticks a nail in their nose then pulls it out or when someone gets a noodle and puts it in their nose and pulls it out of the tear duct.
Your nose is just a bunch of cartilage, plus he's probably had a lot of practice clearing out his cavity.

Well if he has had an injury causing him to have no eye and a hole in his sinus cavity then yes it can be possible for him to do this.
