I'm meeting a woman for dinner tomorrow that I have not met yet. I want to make a good impression. It's a casual dinner, so dress pants or casual khakis? Pullover or buttoned shirt? Dress shoes or casual loafers? Cologne or no cologne? Definitely i'm gonna shave. Anything else?

She should have on a nice dress or skirt and sweater therefore I suggest you choosethe dress pants and a nice shirt which you could wear a nice sweater over and definitely the nice shoes. As far as cologne make sure it is applied lightly and you should be just fine.

khakis, polo, and loafers, cologne but not too strong, yes do shave
