Ask your employer. I'm sure it varies with different companies, unless there is a legal requirement. You don't provide enough information.

This varies by state, most states required "immediate" following accident, but immediate is anywhere up to 72 hours in some states.

Check the form. It is an OSHA form, and there are probably instructions on the back.
