Talk to the priest, youth minister, or religious education director to find out how they do it in your parish. You will need a certain amount of study and preparation and probably some community service.

If you want to accept Jesus Christ as the Lord of your life, all you have to do is believe that He came in the flesh, that He died for your sins, and was resurrected. Repent of your sins...(ask Him to save you and to forgive you for your sins and turn away from those things you know are wrong) and you are saved by your faith in Jesus Christ.
The Holy Spirit will come to live inside of your heart, mind and soul as your Helper...and you will be sanctified (purified, or made holy) for the rest of your life on this earth...becoming a better and better Christian each and every day.
Once you are saved, no one, and I mean NO ONE can take away your salvation and you cannot lose it because Jesus said:
John 10:27-30
27 My sheep listen to my voice; I know them, and they follow me. 28 I give them eternal life, and they shall never perish; no one can snatch them out of my hand. 29 My Father, who has given them to me, is greater than all; no one can snatch them out of my Father's hand. 30 I and the Father are one."
Remember this also, no one can earn this Gift that is from God. No one deserves to be saved, so no one can ever brag that they did something to be saved. We have faith in Christ and that is the only way to be saved.
When we go before God at the Judgment He will see Jesus Christ's Goodness in us, instead of our sins. So it is because of God's Love for His Son that we have eternal life in heaven, and it is because God Loves us so much that He gave His Only Son to pay for our sins so we wouldn't have to pay for our sins in hell.
John 3:16
"For God so loved the world that he gave his one and only Son, that whoever believes in him shall not perish but have eternal life.
Ephesians 2:8-9
8 For it is by grace you have been saved, through faith--and this not from yourselves, it is the gift of God-- 9 not by works, so that no one can boast.
If you want to belong to a specific Catholic church I would recommend going to that church and speaking to a priest.
You can accept Jesus Christ as your Savior before you ever enter or belong to any church...so by all means, do so as soon as you are ready. God is there for you at all times...and yes even this very moment.
God Bless you little brother!
