It is a scam for sure unless you personally applied for a job at that particular medical center in nigeria. There is a scam known as 'phishing' which is done by email over the net promising a huge inheritance if one sends 'x' amount of money, blah blah blah. They tend to be from overseas including nigeria. Whoever emailed you may have gotten your email address from a job website that you posted your resume onto or somewhere else, but I wouldn't trust that "offer" 'as far as I can throw it'.

Did you apply for the job?
Is the english grammar and spelling perfect?
Is the email addressed to you by your name?
If the answers are no, then it is a SCAM.
Usually anything from Nigeria is a SCAM. I would ignore the email, delete it and forget you ever saw it.
Anf if you get any other job offers like that - again you must ignore and delete them.

It's not likely to be true. If they didn't even interview you in person, just made you an offer at some salary that's above what's reasonable, it's DEFINITELY a scam. Accept it, and the demands for money to cover some kind of expense will start.
