Depends on the reasons it didn't work the last time. If someone betrayed the other, or you just weren't compatible, then leave it. But sometimes relationships just kind of fade because life gets in the way. . . . if that's what happened and you still like each other, definitely pursue it.
You don't necessarily have to ask him point-blank to go for it again, just suggest the two of you spend some time together, and see what happens.

If both of you are still into eachother. go for it again. Their is no need to be scared. Unless their was a big reason on why you broke it off. If you fell that he should make the first move and he isn't doing anything then that should give you a hint of how he will be in the future.

Well, if there wasn't anything serious involved in your breaking up, it's still been some time since then, you should probably move on. He may no longer be interested. In the interim, if he does ask you out, it's up to you if you want to try again.

Why did you stop dating in the first place? If it was just casual, and there wasn't a "break up", there's no reason not to ask him out - go for it!
If it was a "break up", it depends whether the issue that split you has been resolved - if yes, give it another try, if no, its time to move on.

I would revisit the reason you stopped dating in the first place. If it's something that has changed and you both can deal, then go for it! If not, just remember that history ususally repeats itself.
I wouldn't be too forward about it, but just drop certain hints so he will know what's up.
