I would say it differently because I wouldn't want to hurt his feelings. I'll ask if he has brushed his teeth yet? Or would you like a piece of gum?
I definitely want someone to do that for me. But usually they say what you've written here and I don't like it. :-(

Of course - part of loving someone is being able to be totally honest. You can be nice about it though - you don't have to be brutally honest.
