You need to be more specific - this question is too vague. Are you asking for proof that your religion is true, or are you asking for proof that your religion exists? Be more specific - get more specific answers.

Do you want the proof so that you know what faith you are or is someone asking for it?

We have the choice to decide what faith we will be, I was raised Southern Baptist but I do not believe in their doctrines so I have changed to a different church.

If someone is asking for "proof" of your religious affiliation, first... it is none of their business. But if you want to provide it you can probably get the minister, priest, rabbi or whomever to write you a letter stating that you are a member of that church... if you are in fact a member.

The only real "proof" of your religion is your actions.

If you mean something that shows what church/temple/mosque you attend, I would imagine a nice inquiry at the office could produce a certificate or some such.

super-charismatic, and then attract a bunch of teenaged
groupies. Brainwash them, and then declare yourself God
to them and that they are "chosen" by you to survive Armageddon, if they'll keep servicing you that is...
Watch them as they become your adoring, slack-jawed, glassy-eyed slaves...
THEN see what happens when someone tell your girlies that you're NOT God, and watch them go into a murder frenzy and throw their futures away over you. (If that ain't proof of at least YOUR religion, what is..?)

Were you baptized or have you officially joined a church? If so, they probably have it on their records. Contact them directly and ask. You can;t necessarily prove you believe something, but you can prove your membership within a religious organization.
