I have a friend who is a ordained reverand, and a Ph.D in theology who has now joined the USAF reserves as a Chaplin. His form of address used to be Rev. Dr. now what will be his approriate form of address to put on his graduation from OTS cake from his Church.

Usually, military chaplains are addressed by their military rank or just as "Chaplain" regardless of religious denomination.
