"Club Cab" and "Quad Cab" are both trade-marked brand names of Chrysler Corp, and apply to Dodge trucks. A Club Cab is the extended cab with a second seat, or jump seats in the back of the cab, but only two doors. A Quad cab is a four-door pickup (also known as a crew-cab.)

On the 94 to 01 extended cab Dodge Rams, club cabs don't have doors on extended cab part and quad cabs do... This feature of the rear opening extended cab doors is only on the 98 to 01 models. Dodge changed the body style in 02 and now only makes single cab and quad cabs with four full doors, no extended cab models anymore....

Jet Doc is correct, George is wrong, Dodge introduced the "Cub Cab" in 1973. Crew cabs usually refer to full size pickups and quad cabs usually refer to compact or mid size pickups, although in recent years the industry has been referring to most four door trucks as quad cabs.
