Does anybody know what is done with this form?

I think it is given to the lawyers, so they can ask you questions, to keep you or eliminate you. I wouldn't take it too personally. They want a fair base for their clients, & wouldn't want an obvious racist, or other major opinion clouding the honesty of a case.

You fill it out and send it back in the envelope provided. In some areas you bring it with you on the day you have been called. The form is used by attorneys who must choose juries for their individual cases. For instance, there are certain people that prosecutors like to have on juries and defense attorneys want to keep off, and visa verse. Prosecutors hate and defense attorneys love having scientists or science teachers on juries because most scientists want reproducible evidence. Housewives and other professions have certain biases that are taken into consideration when choosing a jury.

If you are selected to be part of the pool for a trial, the lawyers for both sides will be given the info. They use it to decide whether or not to include you in the jury for that trial
