Salam

You can never convert to Islam, you where born muslim and so was your pope and all those created from Adam and beyond, in submission and that is Islam and there is no other word in the english langauge that means submission other than submission and that word translates to Islam in arabic so you can call yourself convert if you like but you will be reverting back to your original birth state.

The name you choose means nothing.

No you do not have to. I know a lot of converts that choose not to. And thats fine in the religion.

And just something cool to know:
If a Muslim adopts a child. They are not supposed to change the childs last name to their own. :)

No, you don't have to. Just because a person has an Arabic name, doesn't mean that you are a Muslim, so you don't have to change your name.

No. But you'll probably find that you're greatly inclined to do it as a sign of leaving your previous life as a non muslim behind.
