It seems strange that she got so offended. It's a very formal way to a address somebody, and a lot of times someone will say something along the lines of "I'm not a ma'am," but usually it's a request for less formality, not an objection to rudeness. Occasionally I've seen someone say "yes, ma'am" or "yes, sir" to imply that somebody is being bossy, but it doesn't sound like she interpreted it that way. It's more likely that she was just looking for a reason to tell you off. Maybe she was having a bad day.
